Pay to Participate Refund
If you paid the Pay to Participate Fee and your child did not participate in any club or sport, you may submit our refund form. If you paid the full athletic fee and your child only participated in clubs, you are entitled to a partial refund. (If your child did participate in one sport, you will not be entitled to a refund).

Click the underlined text to print the file and either scan & e-mail it to ALewis@bloomfield.org, fax it to 248-341-6449, or send it via U.S. mail to the address on the form. Please note, this form does not go back to your child's school, it is to be returned to the Athletics, Recreation & Community Services Department.

Your refund will be issued either via check or credit card, depending on how you made the payment originally, however if you paid by credit card and prefer a check refund, there is a spot to indicate that on the form.

I will e-mail you once I received your form, if you do NOT receive a confirmation e-mail from me, please send your form again. Absolutely NO refunds will be given if the form is not received by the deadline date of June 2nd, 2017. No monies are refunded after June 30th, 2017. Your payment made during the 2016/2017 school year is NOT transferable to the next school year.
