Health Risk Assessments are Due 9/15!

Health Risk assessment forms are due no later than September 15. 

To qualify for the rebate, the employee and spouse (if applicable) must submit to an annual physical exam even if the exam date falls between two plan years (i.e., an exam dated August 15, 2023 cannot be used for both 2023 and 2024 plan years).  Forms received after the due date will not be accepted.
