Bloomfield Hills Schools is committed to providing a safe and healthy school environment and this page is a hub for information regarding the physical and mental health resources for parents regarding their students.

On this page you will find access to essential health forms and immunization guidance, along with Medical Action Plans (MAPs), Medical Management Plans (MMPs), and BHS Authorization for Prescription and Non-Prescription Medication, ensuring streamlined processes for maintaining physical well-being within our educational community. Updated forms and action plans are required at the start of every school year and expire on the last day of school.
 
You will also find information with mental health resources, reporting tools, our District therapy dog program, the Sex Education Advisory Board (SEAB), and many other resources.
 

With the requirement to report, we are asking that parents/guardians and staff complete the below Notify the Nurse form to report student and staff cases of communicable diseases.

Please direct any questions that you may have by calling the district nurse at 248-341-5435.

New and updated forms may be submitted to the school that the student will be attending.  If your child has developed a new health condition or, if your child is new to the district and has a medical condition that is dependent on staff support, please contact the school directly or the district nurse, as soon as possible.
